From: I-126

Follow I-126 into Columbia and take the left fork to merge onto Elmwood Ave. Elmwood will dead end into Bull Street. Turn right on to Bull. Take the 5th left onto Taylor street. Providence Hospital will be on your left a few blocks after crossing Harden Street. Turn Left into the Providence Hospital complex onto Bernardin Ave. Pass under the crosswalk and turn left into the parking area. The office is in Medical Building III on the first floor.

From: I-277

Take I-277 into Columbia and it will become Bull Street. Take the 5th left onto Taylor street. Providence Hospital will be on your left a few blocks after crossing Harden Street. From: I-77

Take Exit 12 Forest Drive/Fort Jackson and go towards Columbia (right if coming from the North). Cross Trenholm Road and Beltline Blvd. Providence Hospital will be on your right. Turn Right into the Providence Hospital complex onto Bernardin Ave. Pass under the crosswalk and turn left into the parking area. The office is in Medical Building III on the first floor.

From: West Columbia

Take Blossom Street into Columbia. Turn Left onto Huger. After crossing Gervais Street, take the 3rd right onto Hampton Street. Follow Hampton through town and turn left onto Harden. Take the 1st right onto Taylor Street. Providence Hospital will be on your left. Turn Left into the Providence Hospital complex onto Bernardin Ave. Pass under the crosswalk and turn left into the parking area. The office is in Medical Building III on the first floor.

From: East Columbia

Take Garner’s Ferry Road toward Columbia. Garner’s Ferry will change into Devine Street. After passing Shandon Methodist Church, take the right fork onto Millwood Ave. Cross Gervais Street, and then take a right onto Taylor. Providence Hospital will be on your immediate left. Turn Left into the Providence Hospital complex onto Bernardin Ave. Pass under the crosswalk and turn left into the parking area. The office is in Medical Building III on the first floor.
